What does your dog's typical day/week look like? i.e. How many walks does your dog go on per day/week? How many potty breaks does your dog get? What time do you take them out in the morning? When is the last time you take them out for the night? Please be as detailed as possible.

Is there anything we should be aware of about your dog that was not covered in the questions above? i.e. My dog has a bite history, my dog is possessive over toys, my dog's food has to be fixed a certain way, my dog has hip issues, my dog is deaf/blind, my dog is on medication, my dog has some environmental triggers, my dog isn't comfortable with things going above their head, etc.

Upon completing this form, please email your vaccination records to [email protected] or have your veterinarian send the records to us directly. If you choose to enroll in an in-house payment plan (excluding financing partners such as Affirm Pay, Denefits, FlexxBuy, and PayPal) and are making payment installments throughout the duration of training, your final balance must be paid no later than 72 hours before your dog’s pick-up date. If the balance is not paid by this deadline, your dog will remain in the possession of Restructured Purpose K9 Academy, LLC. Additionally, a boarding fee of $100 per day will apply for every day your dog remains with Restructured Purpose K9 Academy, LLC until the balance and fees are fully paid. Things your dog will need for the board and train: Food for the duration of the board and train, Collar, Leash, I.D. Tags, and anything else that has been discussed for your particular dog.

Lesson Scheduling:
Each lesson should be completed within 7-10 days of the previous lesson. Please inform your trainer of any scheduling conflicts that may impact the timeline of your program.
Cancellations and Rescheduling:
All scheduled private lessons must be cancelled or rescheduled at least 24 hours prior to the scheduled lesson time. Late cancellations and “No-Shows” will be charged at the normal rate. A late fee will be assessed after 10 minutes, and after 15 minutes, the session will be considered a "No-Show."
Payments:
All payments are due 72 hours prior to the start of the service. If payment is not received within this timeframe, the service may be cancelled at the discretion of Restructured Purpose K9 Academy, LLC. Payment plans are available if needed.
Deposits:
A non-refundable deposit is required to book your service. This deposit will be applied toward the total cost of the service, and the amount will vary depending on the selected service. A payment confirmation will be sent once payment has been processed.
Payment Methods:
Payments can be made via Cash, Certified Cashier's Check, Money Order, Venmo, or Zelle.
Liability:
Restructured Purpose K9 Academy, LLC is not responsible for any illnesses, injuries, or reactions that may occur during or after the service.
Authority and Safety:
Restructured Purpose K9 Academy, LLC reserves the right to render services at the discretion of the service provider. The safety of all parties involved is our top priority. Refunds or reimbursements will be reviewed on a discretionary basis.
Dog Training Process:
Dog training is a process, not an instant fix. Success requires a partnership of accountability, consistency, discipline, loyalty, and love. Following this formula will create a lifetime of memories and happiness.
Client Responsibility:
As the owner and/or client, you are solely responsible for maintaining the training and solicited service(s) provided by Restructured Purpose K9 Academy, LLC.
Photography and Videography:
All services are subject to photography and videography for business purposes. Animals involved in services may be used for advertisements for Restructured Purpose K9 Academy, LLC.
